1) Eligibility:

• This gift certificate is exclusively for “New Clients” who have never visited our practice before.

2) Discount Amount:

• You will receive a discount exactly as mentioned on the gift certificate on your first session when you present the gift certificate.3.

3) Redeemable Sessions:

• We have practices in Chicago (Arlington Heights), Ohio (Cleveland), and California (Los Angeles). The gift certificate is valid for a single session at the practice mentioned on the gift certificate itself.

4) Expiration Date:

• Please note that the gift certificate expires as mentioned on the gift certificate itself. Make sure to schedule your session as soon as possible before this date to take advantage of the discount.

5) Booking Your Session:

• To schedule your session, or to find out session availability and fees structure please follow one of the three methods mentioned below:

6) Presentation Requirement:

• You must present the physical or digital copy of this gift certificate at the time of your session to claim the discount.

7) Non-Transferable:

• This gift certificate is non-transferable and can only be used by the recipient whose name appears on the certificate.

8) No Cash Value:

• The discount certificate has no cash value and cannot be exchanged for cash or any other form of payment.

9) Appointment Cancellation:

• If you need to cancel or reschedule your appointment, please notify us at least 24 hours in advance to avoid any cancellation fees.

10) Additional Services:

• Any additional services or products beyond the session are not covered by this gift certificate and must be paid separately.

11) Validity Period Extension:

• In exceptional cases, we may consider extending the validity period of this gift certificate at our discretion. Please feel free to reach out to discuss further.

12) Lost or Stolen Certificates:

• We are not responsible for lost or stolen gift certificates, so please keep it in a safe place.

13) First Come, First Served:

• Sessions are scheduled on a first-come, first-served basis. We recommend booking your appointment well in advance to secure your preferred date and time.

14) Modifications:

We reserve the right, at our sole discretion, to modify or replace these terms and conditions at any time without prior notice. Any changes made will be effective immediately upon posting on our website or providing notice to clients. It is your responsibility to review these terms periodically for updates.
